This is microscopic view of skin tag … Skin tags (acrochordons) are common, benign fibroepithelial polyps characterized by a hyperplastic, often papillomatous epidermis covering a core of loose, vascularized dermal connective tissue. They frequently appear in intertriginous areas (neck, axilla, groin) as soft, pedunculated, flesh-colored papules, associated with aging, obesity, and insulin resistance. Key Pathological Features (Histopathology) Epidermis: Typically shows acanthosis (thickening), papillomatosis, and sometimes hyperkeratosis. Dermis: Composed of loose, edematous connective tissue (fibrovascular core) with thin-walled blood vessels. Elastic Fibers: The dermal core may exhibit a reduction or complete lack of elastic fibers. Variations: While mostly fibrous, some may contain adipocytes (fibrolipoma). Clinical Relevance Diagnosis: Primarily clinical, but histological examination can confirm diagnosis and rule out other lesions. Associations: They are associated with metabolic syndrome, insulin resistance, and sometimes genetic factors like Birt-Hogg-Dubé syndrome. Symptoms: Though benign, they can become symptomatic due to friction (itching, pain, inflammation).
